WELCOME TO
OUR DEMENTIA JOURNEY JOURNAL

Our Dementia Journey Journal, or ODJJ for short, is a resource that helps family/friends in caregiving roles , and health and social care providers strengthen relationships and improve dementia care through shared communication. Developed through a co-design approach with caregivers, health and social care providers, and persons living with dementia, the ODJJ is a non-clinical resource that offers a structured space to document important details—routines, preferences, personal histories, and care insights. By capturing this information, the ODJJ ensures that the person living with dementia is seen, understood, and supported in a way that respects their individuality and enhances their quality of life.

More than just a tool for record-keeping, the ODJJ fosters meaningful connections. It encourages collaboration between caregivers and care teams, helping to build trust, improve communication, and create a more person-centered approach to dementia care. Available in multiple languages and with adaptations for culturally diverse communities, including South Asian and First Nations groups across Canada, the ODJJ is an inclusive and accessible resource designed to support dementia care across different settings.

Support for Caregivers
